The Welsh Kidney Network is working with Digital Health and Care Wales (DHCW) to make it easier for GPs and other healthcare staff to identify people with CKD and prescribe the right treatments to help protect the kidneys and the heart.
Kidney specialists are currently testing new ways of working, for example working more closely with GPs, pharmacists and heart specialists to improve the care provided for people with kidney disease.
